Emergency Blood Transfusion Trial in UK – Lorne Labs

The BBC have reported on a new clinical trial taking place in the UK involving
blood transfusion
procedures for trauma patients. The study involves 22 centres around the UK and the trial is aiming to determine the optimal mix of red blood cells and plasma that should be given in emergency transfusions.

In normal conditions, most UK blood transfusions involve only the red blood cells and only after
blood screening
has taken place to determine the patient’s blood group. But in emergency situations, when blood screening is not possible, trauma teams or ambulance crews will give just the plasma to patients who are bleeding heavily in an attempt to prevent them losing too much blood. Plasma can be given without causing harm to the patient. However, giving the patient the wrong type of red blood cells -from a donor with a different blood group – can sometimes result in the patient’s death. Many of the advances made in this field have been based on the experience of combat troops in Iraq. Doctors there have had to deal with patients who are suffering major trauma and the early use of plasma, in higher volumes, has made a real difference in patient outcomes. The trial in the UK will impact upon hospital readiness procedures for patients being brought in by air and ground ambulance crews.

An emergency transfusion can save a life. The plasma given may prevent excessive and catastrophic blood loss, but without the red cells, the normal functions of blood are unable to work effectively. Once the patient is stabilized, blood screening procedures can help determine the patient’s blood group and more conventional forms of blood transfusion can be administered.

It will be interesting to see the outcome of the trials on survival rates. American trials found that mortality rates amongst US troops fell considerably when changes to procedures were introduced. Time will tell how it will impact upon UK trauma survival rates. In times when an emergency transfusion is required and blood screening is not available, it is heartening to know that advances in these techniques are getting better and better.
